背景
一、病理檢測
“病理”一詞相信大多數人都聽過,但又說不出個所以然。比如常規做的胃腸鏡檢查,如果檢查到有息肉組織,醫生會在切除息肉的同時送部分樣品到病理科做病理檢查,伴隨有一份病理診斷報告--這就是我們生活中接觸的病理組織學檢查及病理診斷。
“病理是診斷的金標準”,那到底什么是病理組織學檢查?對于病理診斷有怎樣的意義呢?
病理組織學檢查是指將通過手術或者內鏡等操作后取下的身體某個部位的病變組織通過一系列的操作,做成病理切片,并經過蘇木素-伊紅(HE)染色,亦或是免疫組織化學染色后供病理醫生在顯微鏡下觀察細胞形態,來判斷組織發生的病變,是病理醫生進行病理診斷判讀的重要依據之一。

二 免疫組化病理檢測
免疫組化,全稱是免疫組織化學(Immunohistochemistry,IHC),是病理診斷中一種重要的輔助檢查技術手段,主要根據抗原抗體反應和化學顯色原理,組織切片中的抗原先和一抗結合,再利用一抗與標記生物素、熒光素等二抗聚合物進行反應,再用標記辣根過氧化物酶(HRP)或堿性磷酸酶(AKP)等的抗生物素(如鏈霉親和素等)與酶標二抗聚合物結合,最后通過呈色反應或熒光來顯示細胞或組織中化學成分,在顯微鏡下可清晰看見組織細胞內發生的抗原抗體反應產物,從而能夠在組織切片上原位確定某些化學成分的分布和含量[2]。

免疫組化在臨床病理診斷中的意義[3]:
(1)惡性腫瘤的診斷與鑒別診斷;
(2)確定轉移性惡性腫瘤的原發部位;
(3)對某類腫瘤進行進一步的病理分型;
(4)確認組織類型和種類;
(5)發現微小轉移灶;
(6)為臨床提供治療方案的選擇。
免疫組化在科研及新藥研發中的應用:
